National Amputation Foundation, Inc. Scholarship

    Logo is a circular seal with the words 'National Amputation Foundation' encircling a square containing symbles surrounding by tw laurel leaves.

    The foundation will award $125 for major limb amputee students studying in any field at any accredited insitution; renewable on an annual basis as long as funds are available. The award is open to high school students who have a major limb amputation and who will be attending college full time as an undergraduate.

    Selection is based on verification of amputation and the deadline is open. To apply send a letter including:

    • Name and address of the college you will be attending.

    • Social security number.

    • Letter from doctor verifying your amputation.
