UCP Services

UCP offers a wide range of services across the country. Browse the links below to find out more about programs in your community.


Transition Services
UCP of Western New York's Children's Services provides transition services to students, 13-21 years of age, who have multiple disabilities. The primary focus is to develop program plans for students that create outcomes for adult participation in all aspects of the community, such as employment, living situations, and leisure activities.

UCP's Children's Services employs a full time transition and work skills teacher. This person's primary function is to:

  • Assist the student's team (parents, teachers, therapists, social workers, etc.) with transition planning that is part of the student's Individual Education Plan (IEP)

  • Provide student assessments

  • Inform families about the transition process and to assist them with their role

  • Assist families with linkage and referral of services

  • Provide students with multiple work training opportunities through partnerships with community businesses and agencies

  • Coordinate partnerships with the Erie County Summer Youth Program to provide summer employment and job training opportunities with the assistance of a job coach
