UCP offers a wide range of services across the country. Browse the links below to find out more about programs in your community.


How Can Service Coordinators Help?

Health Services - Service Coordinators assist in finding a doctor or therapist of your choice, advocate with health care providers, pharmacists, etc. They are also available to accompany people on appointments.

Educational/Vocational - Service Coordinators will arrange transportation, find a new school, day program or job for a person. They also coordinate and advocate with school program personnel.

Living Arrangements - Referrals for residences, and independent living assistance are provided. Assistance in securing subsidized housing, and getting home care and respite services are offered.

Social and Recreational - Extensive assistance is given in finding recreational opportunities such as clubs, summer camps, sports, vacations, leisure programs, and companion programs.

Financial - Service Coordinators assist in applying for and maintaining entitlements such as SSI, food stamps, and Medicaid; as well as assisting in establishing bank accounts and resolving financial problems.

Family- Recognizing the importance of family, UCP Service Coordinators maintain regular contact and support to family members regarding services being provided and the person's well being. Planning is person-centered; not system-centered.
