GENERAL INFORMATION

No specific disability is needed to lease equipment from UCP.

Please bring a copy of your therapist’s recommendation and/or doctor’s prescription. We will also need your most recent tax return or proof of other income like proof of SSI, SSDI, AFDC or Food Stamps.

You must fill out an agreement before leasing or buying equipment.

LEASING

A deposit is needed when leasing any equipment from UCP. The deposit will be based on the customer’s financial status and the current rental value of the equipment.

The customer is allowed to keep the equipment up to a three month period. After the three month period, the customer has the option to purchase the equipment and/or reevaluate the situation with other options.

Since the equipment is previously owned, UCP does not accept responsibility for maintenance on leased equipment.

Once obtaining equipment from UCP, the equipment must be turned in prior to or on the expected date of return.

Any equipment not turned in on the expected date of return will be considered late and the deposit will be forfeited.

PURCHASING

To purchase equipment from UCP, the customer must be in need of the equipment longer than 3 months, or the customer’s health insurance company refuses to pay for any medical equipment. The customer must show proof of denial from their health insurance company.

The purchasing cost of the equipment will be based on the current market value of the equipment and the customer’s financial status.

Once the equipment has been purchased, it has become the property of the customer and the customer is responsible for the maintenance of the equipment.

DONATING EQUIPMENT

UCP is looking for previously owned durable medical equipment that is in good working condition to add to its collection. The organization will accept equipment under the following categories:

• Power Wheelchairs
• Manual Wheelchairs
• Scooters
• Canes and Crutches
• Bath Equipment
• Seating Cushions

Please take a look at our current inventory list.

The program manager has the discretion to accept other forms of medical equipment as well. If the equipment is not in good working condition, a piece of customized equipment, wheelchair lifts and/or bed lifts, it will not be accepted.

IT IS THE CLIENT’S RESPONSIBILITY TO TRANSPORT EQUIPMENT. UCP OF THE MID-SOUTH IS NOT EQUIPPED TO HANDLE PICK-UPS AND/OR DELIVERIES.
