UCP OF SOUTH CENTRAL PA FISCAL /EMPLOYER AGENT

UCP of South Central PA has received numerous commendations and awards for management of service, consumer satisfaction and program longevity for over 40 years. We are among the best quality run programs in the State.

UCP of South Central PA as a provider of Fiscal/Employer Agent Financial Management Services for consumers strongly encourages consumers to exercise diligent and sincere discretion in their capacity to direct their service needs and/or provider(s) of care. In our UCP Role as F/EA service provider, consumers have the ability and power as employer to choose their own direct care worker(s), and independently control with flexibility the direction and scope of their service delivery. UCP of South Central PA supports the consumer, when requested, in provision of the following list of important F/EA services.

• Training and education in regards to responsibilities and duties as a consumer employer. • Training and assistance on the recruiting, interviewing, hiring, and supervisory duties associated with consumer employed workers. • Provision of materials and ideas for consumers to use in training of their workers. • Communication link with the consumer’s Supports Coordinator/Care Manager to more effectively understand service needs. • Assisting the Consumer Employer in completion of the required forms to establish employer identification numbers for federal, state, unemployment insurance, and workers’ compensation insurance agencies. • Preparing and processing of payroll for consumer employed worker(s) through use of appropriate deductions for income tax and FICA withholding, workers’ compensation, and any other tax withholdings required through federal, state, and local regulations. • Obtaining workers’ compensation insurance coverage for attendant worker(s). • Ensuring proper distribution and receipt of consumer employed worker(s) payroll within the proper policy and procedure regulations established by the Home and Community Base Services waivers and the PA Department of Public Welfare. • On behalf of consumers, UCP will handle making payments as necessary and appropriate for federal FICA, state, and local tax withheld, unemployment and workers’ compensation insurance liabilities and other liability payments as required.

ELIGIBILITY

Qualifications for enrollment into the various Home and Community Based Service Programs UCP of South Central PA provides F/EA services for within our six county areas of York, Adams, Franklin, Fulton, Lancaster and Lebanon counties varies. The constant UCP adheres to in our capacity to serve as agent for our consumers with disabilities is our commitment to allowing consumers flexibility as to choice of service workers and consistent support in obtaining quality service delivery. Based on consumer level of care and type of disability, UCP of South Central PA provides F/EA services with the following list of programs and age groups:

• Attendant Care – (ages 18 to 59), and (60 and older) • Attendant Care Act 150 – (ages 18 to 59), and (60 and older) • Independence Waiver – (over 18 years of age) • Obra Waiver – (ages 18 to 60) • PA Department of Aging Waiver – (ages 60 and older) • Commcare Waiver – (21 years of age and older)
