Description
To assist individuals served with daily living skills and community integration. Must regularly lift and transfer individuals served weighing over 50+ pounds.

Requirements
JOB GRADE: Hourly

HOURS OF WORK: As determined by House Manager

QUALIFICATIONS: Reasonable writing skills, willingness to work flexible hours, including nights and weekends (full or part-time), CPR and First Aid Certification required. Employee MUST demonstrate the ability to successfully perform CPR in a kneeling position adjacent to the floor. Must possess high school diploma or GED certificate, valid IL driver’s license and reliable transportation. Must be able to complete training course to become DSP certified and work as needed in the Adult Services and Vocational Programs. Must be at least eighteen years of age.


REPORTS TO: House Manager


JOB DESCRIPTION:

  1. Assist individuals served to maintain a clean and safe living environment through regular housecleaning.
  2. Assist with the preparation of meals, menu planning, grocery shopping and feeding when needed.
  3. Assist with personal hygiene of individuals served including: toileting, dressing, showering, teeth brushing, etc.
  4. Assist individuals served with community access for recreational and consumer activities. Must be trained and able to drive agency vehicles.
  5. Assist the individuals served in the achievement of new skills by offering individual programming and documenting progress.
  6. Assist individuals served with the self-administration and documentation of medications.
  7. Complete all paperwork including daily summaries, incident reports, bowel & bladder charts, seizure reports, etc.
  8. Attend individuals served annual and semi-annual staffings. Provide input on goal progress and on new or revised goals.
  9. Attend staff meetings or training sessions as scheduled.
  10. Other duties as assigned.
