Real Life Choices
Real Life Choices is a funding program provided by the New Jersey Division of Developmental Disabilities that allows eligible individuals to select supports and/or activities on a fee-for-service basis. It is a self-directed approach that provides services and supports that are designed to enhance community living. All services are based on an individual budget and personal preferences.
United Cerebral Palsy offers an array of supports and services that empowers individuals living at home and their families to make decisions about their specific needs. In addition, United Cerebral Palsy will work with families in developing other possibilities for support. Services include, but are not limited to:
-Supported Employment
-Individual Supports/Mentoring
-Day Respite Services
-Hotel Respite Services
DDD eligibility is required.
