Community Services
CSPPPD
CSPPPD is a community living support service specializing in services to non-elderly nursing home residents who have physical disabilities which occurred before age 21. Eligible consumers are targeted during the admissions process. These services include case management, training in community living skills, transportation, encouragement, and other tangible supports needed which enable persons to acquire, regain, improve or maximize their independence, enabling the consumer to move back into the community should he or she choose to do so. For consumers who choose to remain in the nursing home, they may receive services such as case management, advocacy, skills training, community integration, and transportation. For consumers choosing to live in the community, in addition to the previously mentioned services, they may receive attendant care services and assistance in pursuing vocational and educational goals. Also, for consumers living in the community, there is limited funding for minor home modifications and equipment.
Services are provided through a contract with a partner agency in Lancaster funded by the Office of Social Programs, Pennsylvania Department of Public Welfare. Counties Served by UCP Central PA: Cumberland, Dauphin, Juniata, Mifflin, and Perry
