Independence Waiver
The Independence Waiver provides in home, community-based services for daily living assistance and prevents admissions to nursing facilities.
Eligibility:
- Physical disability and functional limitations
- Nursing home level of care
- Over 18 years of age
- Medical Assistance eligible
- Not ventilator dependent
- Total services must be under the cap of the average yearly nursing home costs
Services Provided
- Service Coordination
- Attendant Care Services
- Respite
- Personal Emergency Response
- Physical, Occupational, and Speech therapies
- Visiting Nurse
- Limited assistance with modifications to the home and equipment
Counties Served: Cumberland, Dauphin, Juniata, Mifflin, and Perry
