Susquehanna Attendant Care
Susquehanna Attendant Care service enables eligible adults ages 18 to 59 who are mentally alert and physically disabled to live as independently as possible by performing activities of daily living that they would ordinarily perform themselves were it not for their disability. These include but are not limited to assistance with meal preparation, personal hygiene, activities of daily living and health maintenance activities.
Attendant care is a service which is offered statewide by DPW/OSP. The PA model offers a continuum of care, which enables consumers to choose a model of service from agency management to complete consumer management. Our program serves Juniata, Mifflin, Montour, Northumberland, Snyder and Union counties.
The service consists of Basic, Ancillary and Health Maintenance services provided by attendants in accordance with the consumer’s approved service plan. Types of these services include:
Basic
- Assistance getting in and out of bed, and/or a motor vehicle;
- Assistance with routine bodily functions including but not limited to bathing and personal hygiene, dressing and grooming, eating (including meal preparation and clean up), and health maintenance activities.
If a person is assessed as needing one or more of the basic services above, the following may be provided in conjunction with the basic services:
Ancillary
- Homemaker-type services
- Companion-type services
- Assistance with cognitive tasks
Program Goals
- Enable consumers to live in the least restrictive environment as independently as possible.
- Enable consumers to remain in their homes and prevent unnecessary admission to nursing homes or other similar institutional settings.
- Enable consumers to seek and/or maintain employment.
