Adult Services

Elks Home Service Nurse

The UCP/Elks Home Service Nurse Program provides a Registered Nurse or Medical Care Coordinator to visit children and adults with developmental disabilities in their homes, free of charge.

Referrals

Referrals for services can be made by:
  • Healthcare professionals
  • School personnel
  • Community agencies
  • Family members
  • Friends or concerned Elks
*Permission must first be obtained from the individual or family.

When a referral is received, a visit is made to the home for an intake and initial assessment. The needs of the family and environment are noted at this time.

After discussing the needs with the individual and family, we help to determine the appropriate referrals for services. When referrals to other services are made, we can assist with follow-up. Home visits are made on an as-needed basis.

Our Services

An individual of any age who has a developmental disability that is manifested before a person reaches 22 years old is eligible to receive services, which can be continued throughout a person's lifetime, if needed. In many cases, services provided to children and their families helps ease the transition into adulthood.

Some of our services include:

  • In-home medical care management services
  • Advocating for appropriate educational services
  • Nutritional instruction
  • Assistance with obtaining adaptive clothing and equipment
  • Offering home care instruction
  • Providing emotional support
  • Providing referrals to other community supports
