Supported Living Services
We provide services to participants living in their own home or apartment in the community. Through the assistance of a Coach, individuals receive personalized training and support to facilitate greater independence. Supported Living Services includes emergency, 24-hour, 7-days-a-week, on-call assistance.
Training is provided in an individual's home or in the community in areas of daily living, meal planning/preparation, comparative shopping, household maintenance, medical appointment setting, social skills training and recreation/leisure activities.
You are eligible if you:
- Have a developmental disability
- Are at least 18 years of age
- Express a desire to live in your own home
- And are unable to live in your own home without on-going support
