Case Management

The Case Management program is the entry point for services.

Case Management is a statewide service, providing advocacy, intervention, and assistance with acquiring durable medical equipment, transportation and emergency services to people with disabilities and their families. UCP Case Managers are the way many families and people with disabilities access services and resources. The Case Managers make initial home visits and determine a family’s need for particular services. The Case Manager has experience with different disabilities and has knowledge of the services offered in the community. The Case Manager’s primary responsibility is to connect families to the services and resources they need throughout the state.

The Case Manager also reaches out to the non-disabled community to teach awareness and inclusion. This process is done through outreach within the school and the community as well as providing information to individuals who have a concern about cerebral palsy.
