Case Managment (Decatur/Bloomington)
UCP supports individuals with disabilities living independently in the community or with family by providing case management and training that promotes independence. The case manager assists the individual and family in developing living skills, medical advocacy, family supports, transportation and job-related skills.
Case Management Services can happen at the UCP Building, at community-based sites or at an individual’s residence. Frequency of service depends on the needs of the individual being served.
