In Home Supports
UCP of Central Maryland's in-home support programs are designed to assist individuals who need or want such services as personal care, housekeeping, and case management in order to remain in their own homes. Individual plans are designed by the individual, with family input, to build services and supports based on individual needs and desires. Assistance may be coordinated by the individual or the agency.
Services and supports include, but are not limited to:
- Personal care
- Housekeeping
- Community integration training
- Financial counseling (paying bills, saving, etc.)
- Hiring, firing, discipling care workers
- Emergency assistance
- Location of resources
- Adaptive equipment
- Accessibility modifications
- Assistive technology
- Case management
- Safety
- Habilitation
Community Supported Living Arrangements (CSLA) - Provides supports to adults with developmental disabilities who are in need of long term, on-going supports.
Individual Support Services (ISS) - Provides supports to adults with developmental disabilities who are in need of one time or minimal ongoing supports (e.g. funding to purchase assistive technology or make accessible modifications to one's home; funding to provide personal care to an individual for a few hours each week, etc.)
Family Support Services (FSS) - Provides supports to the family of a child (individual 21 years of age or younger) to enable the family to continue to care for the individual in their home.
*All UCP-CM in-home support programs are fully licensed by the Maryland Office of Health Care Quality.*
